UIL Pre-Participation Physical Evaluation

 

Please read CAREFULLY. The UIL Physical Evaluation requirements have changed for 2020-2021!

This year, everyone must turn in the completed medical history form, but not everyone will need a physician’s signature.

You MAY NOT need a physician’s signature if you have a UIL physical on file with the district from the 2019-2020 school year. Any student who does not have a 2019 UIL physical on file must take the form to a physician for activity clearance. Also, any student who answers YES to QUESTIONS 1, 2, 3, 4, 5, or 6 on the medical history portion, even if they have a 2019 physical on file with the district, must take the form to a physician for a signature.

Follow these steps to submit your completed UIL Physical Evaluation:

1. If you DO NOT need a doctor’s signature, open the editable PDF and complete page 1 of the document (you can disregard page 2). Print the form and sign it at the bottom (student and parent signatures are required). Then scan or photograph the completed, signed form and attach that image to an email to Mr. Dixon at mdixon@csisd.org. Your subject line should be — Last name, First name UIL Physical.

2. If you DO need a doctor’s signature, open the editable PDF and complete page 1 of the document. Print both pages. Take the forms to your doctor’s office for a physical exam and get a signature. Student and parent must sign, as well. Then scan or photograph BOTH pages and attach the image to an email to Mr. Dixon at mdixon@csisd.org. Your subject line should be — Last name, First name UIL Physical.

If you are unable to submit the form by email, please bring the form to summer band or arrange another time to bring it to Mr. Dixon. All students are required to have a current physical on file in order to participate in band and guard.
